What is a GP Chronic Condition Management Plan?
A GP Chronic Condition Management Plan (GPCCMP) is a referral provided by your GP that may allow eligible patients to receive Medicare rebates for allied health treatment services.
Patients with chronic or long-term conditions lasting longer than 6 months may be eligible under Medicare guidelines.




Bulk Billed Allied Health Services Available
We offer fully bulk-billed appointments under GPCCMP for the following services:

You may be eligible if:

You have a chronic medical condition that has lasted 6 months or longer.

Your GP has prepared a GPCCMP referral for allied health treatment.

You haven’t already used your 5 subsidised visits for the current calendar year.
